Statement of Provision forms
For new home educators where the home education provision has not previously been reviewed by the Barnet Education & Learning Service (BELS) Elective Home Education (EHE) team: Initial review of my child’s elective home education provision.
For current home educators whose home education provision has previously been reviewed by Barnet Education & Learning Service EHE team:
Annual update of my child’s elective home education provision.
Other ways of providing information to us about your child’s home education provision
You are also welcome to update us in any other format of your choice e.g. by email, by providing: your own report, a tutor or tutorial centre report, a letter of confirmation from a 14-16 EHE provision, a letter of confirmation from an online school etc. You can also meet with us in person or on Teams to undertake a review of your child’s elective home education provision.
Please ensure that any information provided includes details of your child’s English and Mathematics, including what they have been studying and the resources used. In addition to the progress that your child has made in English and Mathematics whilst home educated (since the last review or since commencing home education – whichever is most relevant.)
Please include details of your child’s day to day socialisation activities (with children and young people outside of the family) and also confirm that your child has the opportunity to work without distractions e.g. at a desk/ work station etc. in a quiet area of your home, at the library etc.
Kindly indicate any SEND that your child may have.
Please also update us on any other changes to your child’s education and any of your contact details e.g. home address, mobile number and email address.
